In the event that a patient cannot personally attend and authorizes another person to collect their medical report, prescription, orthopedic voucher, extract from their medical documentation, or any other document containing their personal information, it is essential that this person is listed on the Patient Request with the provision of personal information in the manner designated by the patient and is aware of the patient’s communication code. Otherwise, the requested documents can only be issued based on an officially certified power of attorney.
